Sending your child to school with medical needs:

If your child has any medical conditions please do not hesitate to speak with your child's teacher, principal, AND school nurse.  The School Nurse is available to help assess what your child needs to succeed at school and we can develop Individualized HealthCare Plans or Emergency Care Plans for them.  Below are some quick references for common medical issues, but we understand that our students have many other medical needs beyond those listed here.

For the student with Asthma

  • Please schedule an appointment to talk with your child's teacher, principal and school nurse.
  • If your child will have an inhaler in the office please provide the school secretary with a physicians order for their medication.  You will also have to sign a permission form so we can administer the medication at school.
  • If your child will be carrying their own medication please have your doctor sign the MT Authorization to Self Carry and Self Administer Asthma Medication form and give a copy to the school to keep on file.
  • Does your child's asthma interfere with their daily life?  The Montana Asthma Program is a great resource that helps your child learn about and control asthma.  Please contact the Montana Asthma Program Home Visiting Nurse at 406-395-4305. If you would like more information about the Montana Asthma Program please click on this link.
  • The school nurse likes to have an Asthma Action Plan on board.  Please print and have your child's doctor complete for the safety of your child.

For the student with life threatening allergies

  • Schedule a meeting with your child's teacher, principal and school nurse.
  • The school nurse will make a Individualized Health Care Plan for your child, this is a team effort, parents must be involved.
  • If you will have an Epinephrine auto-injector kept at school for your child please make sure that the office has a copy of a doctor's order for ALL medication.
  • If your child will be self-carrying their Epinephrine auto-injector please have your medical provider complete the MT Authorization to Self Carry and Self Administer Allergy Medication form to keep on file at your child's school.
  • Please contact the school nurse so she can make a Individualized Health Care Plan or Emergency Care Plan for your child.

For the student with Diabetes

  • Diabetic Medical Management Plan (to be completed with child's physician)
  • Schedule a meeting with your child's teacher, principal and school nurse.
  • The school nurse will make an Individualized Health Care Plan for your child, this plan is a team effort, parents must be involved.

For the student with Seizures

  • Schedule a meeting with your child's teacher, principal and school nurse.
  • A Seizure Care Plan must be complete, this is important so all staff knows what to do in the event of a seizure with your child at school.
  • Please bring documentation to school of your child's seizure diagnosis.
